02.HOUSING NAVIGATION
If you are at risk of losing your home — or have already lost it — we can help you navigate the system. We meet you where you are, not where we wish you were. • Emergency rental assistance applications • Shelter and transitional housing connections • Section 8 and VASH (Veterans Affairs housing) navigation • Coordination with local housing authorities and shelters Homelessness wears work uniforms. 03.FOOD & BASIC NEEDS
You cannot focus on your health when you are worried about your next meal. We connect you directly to resources so you can breathe again. • SNAP enrollment assistance • Food bank and community food source connections • Hygiene products and clothing • Medication access programs
04.MENTAL HEALTH REFERRALS
We do not offer mental health counseling directly, but we will make sure you are never handed a phone number and left alone. We walk alongside you to the right support. • Warm referrals to licensed mental health providers • Substance use and addiction support connections • Trauma-informed care coordination
05.MEDICATION ACCESS
No one should skip a dose because they cannot afford a prescription. We connect you to programs that help cover the cost of medications — including HIV treatment and prevention. • Patient assistance program navigation • PrEP medication support • Pharmacy partnerships including Walgreens and CVS • Direct Relief medication access
06.EDUCATION & SCHOLARSHIPS
Education is the first step toward lasting stability.
